कोरोना से बचाव कैसे करे ?

1.        क्या मुझे कोविड 19 टीका लगवाना चाहिए है ?

1.        हमारे इलाके से कोविड 19  अब गया है। बहुत सारे लोग बिना मास्क के बेखौफ घूम रहे है। उन्हे कुछ नही हुआ। मुझे भी पिछले पूरे साल मे कोविड 19  नही हुआ।  फिर मै टीका क्यों लगाऊँ ?

2.        टीका लगवाने के बाद भी किसी किसी को कोविड – 19 हुआ है। किसी को  पहले नही हुआ था, लेकिन टीका लगवाने के कुछ दिन बाद ही हो गया। फिर क्यों लगवाये ?

3.        क्या यह टिकाकरण पूरी तरह से असरदार और सुरक्षित है ? कई तद्न्य व्यक्तियों ने इसके बारे मे प्रशासन की आलोचना की है, वह यूही तो नही की होगी !

4.        अगर ब्लड प्रेशर डायबिटीस, दिल की बिमारी, अस्थमा या ऐसी कोई बिमारी हो या एन्जिओप्लास्टी जैसा कोई ऑपरेशन हुआ हो तब टीका लगवाने मे खतरा तो नही है ?    

5.        सुना है कि कुछ लोगों को टीका लगाने के बाद किसी प्रकार की तकलीफ हो जाती है। ऐसी हालत मे क्या करना चाहिये?

6.        टीका कराने के बाद भी कोविड – 19 होने की संभावना रहती है, मास्क पहनना बंद नही कर सकते, तो फिर टीका लगाने से क्या फायदा ? 

2.        मेरा कोरोना टेस्ट पॉजिटिव आ गया है, अब मुझे क्या करना चाहिए? कोरोना-साथियों से कैसे बचें?

1.        मेरी कोरोना जाँच रिपोर्ट पॉज़िटिव आ गयी है। क्या अब मै मर जाउंगा ?

2.        कोविड – 19 का फैलाव कैसे होता है?

3.        इससे बचने के लिये 6 फूट की दूरी क्यों जरुरी बतायी गयी है ?

4.        मास्क पहनने से बचाव कैसे हो जाता है ?

5.        मेरे नजदीकी व्यक्ति को बुखार या खांसी नही है, फिर भी उससे 6 फूट की दूरी क्यों बनायी रखनी चाहिये ?

6.        अगर किसी व्यक्ति को बुखार या खांसी जैसी कोई तकलीफ नही है, और वह किसी कोविड – 19 मरीज से मिले, तो क्या उस मरीज से दूसरों को कोविड – 19 की बीमारी हो सकती है ?

7.        कोविड – 19 की बीमारी मुझे और किस तरह से हो सकती है ?

8.        कोविड – 19 वाइरस मेरे आस पास किसी चीज पर हो तो उससे अपना बचाव कैसे कर सकते है ?

9.        कोई खाने की चीज बाहर से लायी गई है क्या उससे हमे कोविड – 19 की बीमारी हो सकती है ?

10.      क्या शराब पीने से बदन के अंदर का कोविड – 19 का वाइरस मर जायेगा?

 

 

क्या कोविड – 91 टीकाकरण बहुत जरूरी है ?

कोविड – 19 टीकाकरण के बारे मे लोगों के मन मे कई आशंकाए है। लेकिन सच यह है कि 60 साल के उपर जिनकी आयु हैं उन सभी को और 45 – 60 साल के बीच जिनकी आयु हैं  व खास करके जिन्हे ब्लड प्रेशर, डायबिटीस, दिल की बिमारी, अस्थमा या ऐसी कोई बिमारी हो या मोटापा हो, उन्हे यह टीका जरूर लगवाना चाहिये और जल्दी लगवाना चाहिये। क्योंकि उनमे कोविड – 19 गंभीर रूप धारण कर सकता है या जान को भी खतरा होने की ज्यादा सम्भावना होती है। उसके बाद बाकी सब आते है। क्योंकि 18 से 45 साल के बीच जिनकी आयु हैं, उन लोगों को काम- काज के सिलसिले मे घर के बाहर ज्यादा आना जाना पड़ता है। इसलिये इस गंभीर बीमारी से बचने या जान के खतरे को कम करने के लिए यह जरूरी है।

अब टीकाकरण के बारे मे जो आशंकाए होती है, उन पर गौर करेंगे:

1.        ‘हमारे इलाके से कोविड – 19 अब गया। बहुत सारे लोग बिना मास्क के बेखौफ घूम रहे है। उन्हे कुछ नही हुआ। मुझे भी पिछले पूरे साल मे कोविड – 19 नही हुआ। फिर मै टीका क्यों लगाऊ?’

जनवरी महीने मे कई लोगों को ऐसा लग रहा था, कि अब कोरोना चला गया है। लेकिन सच यह है कि फिलहाल पूरे देश मे कोविड – 19 महामारी ने गंभीर रूप से अपने पैर पसारे हैं । जिन बस्तियों में नये केस आना बंद हो गये था, वहाँ भी नये केस आ रहे है। ये वो लोग है, जिन्हे पिछले पूरे साल मे कोविड – 19 नही हुआ था। इसका मतलब है कि सबको टीका लगाना चाहिये।

2.        ‘टीका लगवाने के बाद भी कुछ लोग कोविड – 19 से पीड़ित हुये हैं। किसी को पहले नही हुआ था, लेकिन टीका लगवाने के कुछ दिन बाद ही हो गया। फिर मैं क्यों क्यू लगवाएँ?’

पहली बात यह है कि टीके की दोनों खुराक लेने के दस – पंद्रह दिन बाद ही हमारे शरीर मे बीमारी से सामना करने की पूरी शक्ती (प्रतिकारशक्ती) आती है। इस दौरान अगर कोविड – 19 से संक्रमित व्यक्ती से हमारा नजदीकी संपर्क हो गया तो ह्मे भी संक्रमण हो सकता है। टीकाकरण के बाद कोविड – 19 से संक्रमित होने के उदाहरण दिये जाते है, इन मामलों ऐसे लोगों की संख्या बहुत कम होती हैं जिन्होने टीके की दोनों खुराक ले ली हो। टीकाकरण के तुरंत बाद कोविड – 19 से संक्रमित हो जाने के जिन मामलों की बात की जाती हैं, उनकी वजह ये हो सकती है कि टीकाकरण के लिये टिकाकरण स्थल पर बहुत भीड़ रही हो, पर्याप्त शारीरिक दूरी नही रखी गई हो, लोगों ने मास्क नही पहने हो या गलत तरीके से पहना हो और जोर जोर से बाते कर रहे थे आदि। लेकिन इन सभी बातों के आधार पर इसका टीका ना लेना सही नही है, टीका भले ही कुछ देर बाद ले, लेकिन लेना जरूरी है। 

दुसरी बात यह है कि कोई भी टीका बीमारी से 100% संरक्षण नही देता हैं। इस विषय मे जो अध्ययन किये गए है, उससे यह है कि अगर टीके कि दोनों खुराक लेने के बाद किसी को कोविड – 19 हो गया तो संक्रमण गंभीर होने कि संभावना बहुत कम होती है और उस स्थिति मे कोविड – 19 संक्रमण से किसी की मौत होने की संभावना नही होती हैं। जैसे की रेनकोट या छाते से हम बारिश मे पूरी तरह से बच नहीं सकते, लेकिन पूरी तरह भीगते भी नही हैं। 

3.        क्या यह टिकाकरण पूरी तरह असरदार और सुरक्षित है? कई विशेषज्ञों ने इसके बारे मे प्रशासन की आलोचना क्यों की है?

Towards an expanded involvement of public and private sector in production and R&D to meet India’s vaccine requirements

Statement calling for Newer Strategies for Vaccine Production and R&D

 

With the present Indian population of over 130 crores, the number of vaccines required to impart vaccines would be about 310 crore doses (3.1 billion doses), with provisions of about 15% process losses, which is inevitable in any vaccination strategy. While it is not an easy task to gather such huge doses in a short time, but India should not give up. Today therefore the ordinary citizens are seeking answer to this question that why India, a pioneer in vaccine production before the current pandemic and a big exporter of vaccines, has to rely on just two private domestic manufacturers--Serum Institute of India (SII) and Bharat Biotech--to produce Covid-19 vaccines.

 

Presently India has a number of public and private sector units that can make a contribution to the expansion of local production of vaccines. Presently two vaccines namely COVISHIELD of Serum Research Institute (SII), Pune and COVAXIN of Bharat Biotech, Hyderabad are available for supply in India. COVAXIN technology is a home grown technology. Bharat Biotech developed COVAXIN in collaboration with the National Institute of Virology (NIV), which is a public sector R&D institute affiliated to the Indian Council of Medical Research (ICMR), an agency under the control of Ministry of Health and Family Welfare. Presumably NIV and Bharat Biotech jointly own the intellectual property (IP) in the case of COVAXIN. The central government is entitled to make use of the march-in-rights available to government entities. ICMR has been a public funding source for COVAXIN. In the case of COVAXIN, as ICMR is a co-owner of IP, the government can nudge Bharat Biotech to handhold the interested public and private sector units.

 

In the beginning of 2000s, 80% of India’s vaccines for the Universal Immunization Programme were sourced from the public sector. Today, 90% are sourced from the private sector at a higher cost. Brazil, Cuba and China are using public sector companies and institutes to undertake integrated R&D and production operations to vaccinate their populations and export to developing countries to meet their requirements. India has neglected public sector units. India has a large number of a few decade old facilities as well as new facilities equipped with appropriate modern infrastructure. The central and state governments should be making full use of all these facilities to expand local production of COVID vaccines.  Presently India has eleven public sector units. Some are almost ready to go into production. The government has taken some initial steps in the direction of using a few selected units. Integrated Vaccine Complex needs just one hundred crores and some handholding to start the local production of COVID vaccines.

 

Almost a similar number of private sector units exist which can also contribute to the local production of COVID vaccines. There are quite a few efficient Indian Private Sector Vaccine Manufacturing units , such as Biological E. Limited, Hyderabad ; Panacea Biotech Malpur, Solan, Himachal Pradesh ( Head Office in Delhi). Procurement of already approved vaccines from abroad by private sector units is also an option. Already, Dr. Rdddy’s Lab has teamed up with Russia to procure the Sputnik –V Vaccines in the country. More such entrepreneurs coming; this would ease the Vaccines availability. Altogether this way there are close to twenty-two units which can be involved in the production of COVID vaccines. Domestic as well as export requirements can be met to the fullest with the expansion of local production with the involvement of these public sector undertakings and private sector units.

 

While now the private sector is itself getting subsidies and advance funding from the government, the public sector is still not getting the required support. Recently only the government grants became available to state owned companies – Indian Immunologicals Ltd. Hyderabad and Bharat Immunologicals and Biologicals Corporation Ltd. (BIBCOL) Bulandhshahar, UP, to manufacture Covaxin under license. Nevertheless it will take nearly a year for them to produce the first batch of Covaxin. Although the central government has acceded to the Maharashtra Chief Minister’s demand for involving the Haffkine Institute, a state PSU, has received a government grant of 81 crores to get started with the manufacturing of Covid-19 vaccine. Haffkine requires technology transfer. SII and Bharat Biotech should be made to handhold these units to pay back their own long standing debt to public sector. India’s private sector is a child of the Indian public sector units.

 

Here are our specific suggestions for the government to bring out a wholesome policy within this month itself to give a boost to the local production of vaccines. These suggestions are as follows:

 1. Revive public sector undertakings and state owned enterprises to ramp up the vaccine production;

2. Reissue the tender for use of IVC facility with clear provisions allowing the state governments, public sector undertakings and state owned enterprises to plug and play with the facility;

3. Issue compulsory licenses to interested parties to allow them to enter into production of COVID 19 vaccines;

4. Make use of the conventionally used march-in- rights available to ICMR to direct Bharat Biotech to  share know-how with the PSUs and SOEs to allow them to leverage the joint IP provisions for the acceleration of production of vaccines for domestic use by the central and state governments;

5. Direct and mobilize the SII owners to share the IP and rights to know-how or to initiate joint ventures (JVs) in collaboration with the interested public and private sector entities for the use in local production of vaccines; 

6. Ask the state governments to take steps to ensure funding by issuing the advance purchase commitments to the JVs in making to accelerate the production of vaccines with the objective of ramping up COVID inoculation for the use of vaccines by the state and centre for the population below 45.

7. Research on new vaccine development strategies and development of multiple
vaccines be enabled and encouraged across research laboratories, public sector
and private sector institutions.
These scientific efforts need to be encouraged in every possible way. Newer versions of COVAXIN need to be developed by the BHARAT BIOTECH in collaboration with NIV, ICMR in India to increase the efficacy.

खेती की लागत बढ़ने, जलवायु में बदलाव के चलते होने वाले खतरों से और बाजार की दोषपूर्ण नीतियों से घिरे हुए किसान खुद को बचाने के लिए संघर्ष कर रहे हैं। सेंटर फॉर साइंस एंड एनवायरमेंट की महानिदेशक सुनीता नारायण का कहना है कि किसानों का साथ दिया जाना चाहिए। 

भारत में हर दिन 28 से ज्यादा किसान और खेतिहर मजदूर आत्महत्या करते हैं। यह कहना है डाउन टू अर्थ मैगजीन और सेंटर फॉर साइंस एंड एनवायरमेंट द्वारा संयुक्त रूप से जारी की गई इस साल की रिपोर्ट स्टेट ऑफ इंडिया'ज एनवायरमेंट' का। इस रिपोर्ट को आज एक ऑनलाइन कार्यक्रम में देशभर के 60 पर्यावरण विश्लेषकों व कार्यकर्ताओं, पत्रकारों और शिक्षाविदों की उपस्थिति में जारी किया गया।

रिपोर्ट के मुताबिक, 2019 में 5,957 किसानों और 4,324 खेतिहर मजदूरों ने आत्महत्या की। 2018 में ये आंकड़े क्रमशः 5,763 और 4,586 थे। 

रिपोर्ट कहती है कि 2019 में 17 राज्यों और केंद्र शासित प्रदेशों में किसान द्वारा आत्महत्या के मामले दर्ज किए गए। 2018 में 20 राज्यों में किसानों के आत्महत्या के मामले दर्ज किए, जबकि इसी साल 21 राज्यों में खेतिहर मजदूरों ने आत्महत्या की। यह आंकड़ा 2019 में बढ़कर 24 हो गया।

कुल मिलाकर, 2019 और 2018 के बीच नौ राज्यों में किसान आत्महत्या में इजाफा देखा गया। ये नौ राज्य हैं - आंध्र प्रदेश, असम, छत्तीसगढ़, हिमाचल प्रदेश, महाराष्ट्र, मिजोरम, पंजाब, उत्तर प्रदेश व अंडमान व निकोबार द्वीप समूह।

डाउन टू अर्थ के प्रबंध निदेशक रिचर्ड महापात्रा पूछते हैं कि' "हमारे किसान अपनी जान क्यों ले लेते हैं? राष्ट्रीय अपराध रिकॉर्ड ब्यूरो किसानों की आत्महत्या के कारणों को लेकर चुप है, जबकि 2016-2017 में आई एक सरकारी रिपोर्ट इसके पीछे तीन कारण बताती है- मानसून की अनियमितता के चलते फसल की बर्बादी, सिंचाई के लिए पानी की सुनिश्चित सप्लाई न होना और फसल पर कीड़ों व अन्य बीमारियों का हमला। लेकिन इन सब मौतों के पीछे जो असली कारण है वो ये है कि कृषि अब बेहद ही घाटे का सौदा बन गया है। बाज़ार किसानों को उनका सही दाम नहीं देता है।"

एक हालिया लेख में सुनीता नारायण ने बेहद विस्तार से उन कुरीतियों पर चर्चा की है, जिन्होंने कृषि को बुरी तरह प्रभावित किया है। एक तरफ जहां दुनियाभर के देश अपने यहां के किसानों और कृषि का सहयोग करते हैं और उनपर पैसा लगते हैं, वहीं भारतीय किसान इस तरफ से नुकसान में रहते हैं।

नारायण ने कहा, "भारतीय किसानों को वो आर्थिक सहयोग नहीं मिलता, जिससे कृषि को लाभदायक बनाया जा सके। इसके साथ ही, बीज से लेकर पानी और मजदूरी तक की लागत बढ़ रही है, जलवायु परिवर्तन के चलते बेहद खराब मौसम देखने को मिल रहा है। जब फसल महंगी हो जाती है तो सरकार विदेश से सस्ता अनाज आयात कर लेती है। इसके चलते हमारे किसान दोनों तरफ से कष्ट उठाते हैं। यही कारण है कि किसान न्यूनतम समर्थन मूल्य (एमएसपी) की मांग कर रहे हैं, ताकि कीमतों में बदलाव होने के बावजूद वे सुरक्षित रह पाएं। 

उन्होंने इस तरफ भी ध्यान आकर्षित किया कि मौजूदा एमएसपी व्यवस्था दोषपूर्ण है। भले ही 22 फसलों के लिए एमएसपी तय की गई है लेकिन इसका इस्तेमाल सिर्फ गेहूं और चावल जैसी कुछ फसलों के लिए होता है जिनके लिए सरकार ने खरीद व्यवस्था बनाई है। इतना ही नहीं, सरकार के अपने आंकड़े बताते हैं कि 600 थोक बाज़ारों में 10 चुनी हुईं फसलों के तकरीबन 70 फ़ीसदी लेन-देन एमएसपी से कम पर हुए हैं। 

नारायण में कहा कि, "भारत कई साल से गंभीर कृषि संकट से जूझ रहा है और इसका दुखद परिणाम हम देख रहे हैं किसानों व खेतिहर मजदूरों द्वारा की जा रही आत्महत्यायों के रूप में। यह समय है कि हम जो भोजन कर रहे हैं उसके असल दाम के बारे में बात करें और जो किसान हमारा भोजन उगा रहे हैं उनके फायदे के बारे में सोचें।"

इम्यूनिटी यानी प्रतिरक्षा शब्द जितना विज्ञान में इस्तेमाल होता है , उससे कहीं अधिक आम बोलचाल में। वैज्ञानिकों और डॉक्टरों से अलग आम सामान्य जन इन शब्दों को ढीले-ढाले ढंग से प्रयोग करते हैं। आपको बार-बार ज़ुकाम होता है ? लगता है आपकी प्रतिरोधक क्षमता कम है! आप को कमजोरी महसूस होती है? डॉक्टर से अपनी इम्यूनिटी की जाँच कराइए और पूछिए कि इम्यूनिटी बढ़ाने के लिए क्या खाएं, कैसे रहें , कैसा जीवन जिएं! 

 
इम्यूनिटी को समझने के लिए शरीर के एक मूल व्यवहार को समझना जरूरी है। सभी जीवों के शरीर निज और पर का भेद समझते हैं। वे जानते हैं कि क्या उनका अपना है और क्या पराया। शरीरों के लिए अपने-पराये की यह पहचान रखनी बेहद जरूरी होती है। अपनों की रक्षा करनी है, परायों से सावधान रहना है। जो पराये आक्रमण करने आये हैं --- उनसे लड़ना है , उन्हें नष्ट करना है। 
 
शरीर का प्रतिरक्षक तन्त्र यानी इम्यून सिस्टम इसे अपनत्व-परत्व के भेद को बहुत भली-भांति जानता है। उदाहरण के लिए, मनुष्य के शरीर की प्रतिरक्षक कोशिकाओं को पता चल जाता है कि अमुक कोशिका अपने रक्त की है और अमुक बाहर से आई जीवाणु-कोशिका है।

फिर वह अपने परिवार की रक्त-कोशिका से अलग बर्ताव करता है और बाहर से आयी जीवाणु-कोशिका से अलग। यह भिन्न-भिन्न बर्ताव प्रतिरक्षा-तन्त्र के लिए बेहद जरूरी है। जब तक पहचान न हो सकेगी , रक्षा भला कैसे होगी !
 
प्रतिरक्षा-तन्त्र को लोग जितना सरल समझ लेते हैं, उससे यह कहीं बहुत-ही ज्यादा जटिल है। इम्यूनिटी किसी एक वस्तु को खाकर या न खाकर नहीं बढ़ायी जा सकती और न कोई एक अच्छा-बुरा आचार उसके लिए जिम्मेदार ही है।

प्रतिरक्षा-तन्त्र में अनेक रसायन हैं, भिन्न-भिन्न प्रकार की कोशिकाएं हैं। इन सब का कार्य-कलाप भी अलग-अलग है। यह एक ऐसे हजार-हजार तारों वाले संगीत-यन्त्र की तरह जिसके एक तार को समझकर या बजाकर उत्तम संगीत न समझा जा सकता है और न बजाया ही। 
 
जटिलता के अलावा प्रतिरक्षा-तन्त्र का दूसरा गुण सम्पूर्ण शरीर में व्याप्त होना है। प्रतिरक्षक कोशिकाएं हर जगह गश्त लगाती हैं या पायी जाती हैं: रक्त में, त्वचा के नीचे , फेफड़ों व आंतों में, मस्तिष्क व यकृत में भी। इस जटिल सर्वव्याप्त तन्त्र के दो मोटे हिस्से हैं: पहला अंतस्थ प्रतिरक्षा-तन्त्र और दूसरा अर्जित प्रतिरक्षा-तन्त्र। इम्यून सिस्टम के इन दोनों हिस्सों को समझकर ही हम इसके कार्यकलाप का कुछ आकलन कर सकते हैं।
 
अन्तःस्थ का अर्थ है जो पहले से हमारे भीतर मौजूद हो। अंग्रेजी में इसे इनेट कहते हैं। प्रतिरक्षा-तन्त्र के इस हिस्से में वह संरचनाएं, वह रसायन और वह कोशिकाएं आती हैं , जो प्राचीन समय से जीवों के पास रहती रही हैं। यानी वह केवल मनुष्यों में ही हों, ऐसा नहीं है; अन्य जीव-जन्तुओं में भी उन-जैसी संरचनाएं-रसायन-कोशिकाएं पाई जाती हैं, जो संक्रमणों से शरीर की रक्षा करती हैं।
 
उदाहरण के तौर पर हमारी त्वचा की दीवार और आमाशय में पाए जाने वाले हायड्रोक्लोरिक अम्ल को ले लीजिए। ये संरचना और रसायन अनेक जीवों में पाये जाते हैं और इनका काम उन जीवों को बाहरी कीटाणुओं से बचाना होता है। इसी तरह से हमारे शरीर के मौजूद अनेक न्यूट्रोफिल व मोनोसाइट जैसी प्रतिरक्षक कोशिकाएँ हैं। ये सभी अन्तस्थ तौर पर हम-सभी मनुष्यों के भीतर मौजूद हैं।
 
अन्तःस्थ प्रतिरक्षा-तन्त्र सबसे पहले किसी कीटाणु के शरीर में दाखिल होने पर उससे मुठभेड़ करता है। पर यह बहुत उन्नत और विशिष्ट नहीं होता। इस तन्त्र की कोशिकाओं की अलग-अलग शत्रुओं की पहचान करने की ट्रेनिंग नहीं होती।

शत्रु को मुठभेड़ में नष्ट कर देने के बाद ये कोशिकाएं इस युद्ध की कोई स्मृति यानी मेमोरी भी नहीं रखतीं। इन सब बातों को ध्यान में रखते हुए अब तनिक प्रतिरक्षा-तन्त्र के दूसरे हिस्से अर्जित प्रतिरक्षा-तन्त्र को समझिए। 
 
अर्जित का अर्थ होता है अक्वायर्ड। वह जो हमारे पास है नहीं , हमें पाना है। वह जो विरासत में नहीं मिला , बनाना पड़ेगा। प्रतिरक्षा-तन्त्र का यह अधिक उन्नति भाग है। इसके रसायन और कोशिकाएँ विशिष्ट होते हैं , यानी ख़ास रसायन और कोशिकाएं खास शत्रु-कीटाणुओं से लड़ते हैं। प्रत्येक किस्म के कीटाणुओं के शरीर में प्रवेश करने पर खास किस्म की प्रतिरक्षक कोशिकाओं का विकास किया जाता है, जो कीटाणुओं से लड़कर उन्हें नष्ट करती हैं। लड़ाई में इन कीटाणुओं को हारने के बाद ये कोशिकाएं अपने भीतर इन हराये गये कीटाणुओं की स्मृति रखती हैं, ताकि भविष्य में दुबारा आक्रमण होने पर और अधिक आसानी से इन्हें हरा सकें। लिम्फोसाइट-कोशिकाएँ अर्जित प्रतिरक्षा-तन्त्र की प्रमुख कोशिकाओं का प्रकार हैं। 
 
अन्तःस्थ और अर्जित , प्रतिरक्षा-तन्त्र के दोनों हिस्से मिलकर के शत्रु-कीटाणुओं से लड़ते हैं। किसी संक्रमण में अन्तःस्थ प्रतिरक्षा अधिक काम आती है , किसी में अर्जित प्रतिरक्षा , तो किसी में दोनों। इतना ही नहीं कैंसर-जैसे रोगों में भी प्रतिरक्षा तन्त्र की कोशिकाएं लड़कर उससे शरीर को बचाने का प्रयास करती हैं। कैंसर-कोशिकाएं यद्यपि शरीर के भीतर ही पैदा होती हैं , किन्तु उनके सामने पड़ने पर प्रतिरक्षा-तन्त्र यह जान जाता है कि ये कोशिकाएं वास्तव में अपनी नहीं हैं , बल्कि परायी व हानिकारक हैं। ऐसे में प्रतिरक्षा-तन्त्र कैंसर-कोशिकाओं को तरह-तरह से नष्ट करने का प्रयास करता है। 
 
वहीं, अर्जित प्रतिरक्षा-तन्त्र का विकास संक्रमण से हो सकता है और वैक्सीन लगा कर भी। संक्रमण से होने वाला विकास प्राकृतिक है और टीके ( वैक्सीन ) द्वारा होने वाला विकास मानव-निर्मित होता है। 
 
वर्तमान कोविड-19 पैंडेमिक ( वैश्विक महामारी ) एक विषाणु सार्स-सीओवी 2 के कारण हो रही है। इस विषाणु के शरीर में प्रवेश करने के बाद प्रतिरक्षा-तन्त्र के दोनों हिस्से अन्तःस्थ व अर्जित प्रतिरक्षा-तन्त्र सक्रिय हो जाते हैं। वे विषाणुओं से भरी कोशिकाओं को तरह-तरह से नष्ट करने की कोशिश करते हैं। चूंकि यह विषाणु नया है , इसलिए ज़ाहिर है कि अन्तःस्थ प्रतिरक्षा-तन्त्र इससे सुरक्षा में बहुत योगदान नहीं दे पाता। ऐसे में अर्जित प्रतिरक्षा तन्त्र पर ही यह ज़िम्मा आ पड़ता है कि वह उचित कोशिकाओं व रसायनों का विकास करके इस विषाणु से शरीर की रक्षा करे।  
 
मनुष्य के प्रतिरक्षा-तन्त्र के लिए यह संक्रमण नया है , वह उसे समझने और फिर लड़ने में लगा हुआ है। ऐसे में उचित टीके ( वैक्सीन ) के निर्माण से हम प्रतिरक्षा-तन्त्र की उचित ट्रेनिंग कराकर अर्जित प्रतिरक्षा को मजबूत कर सकते हैं। उचित प्रशिक्षण पायी योद्धा-कोशिकाओं के पहले से मौजूद होने पर शरीर के भीतर जब सार्स-सीओवी 2 दाखिल होगा , तब ये कोशिकाएँ उसे आसानी से नष्ट कर सकेंगी। किन्तु सफल वैक्सीन के निर्माण व प्रयोग में अभी साल-डेढ़ साल से अधिक का समय लग सकता है , ऐसा विशेषज्ञों का मानना है। 
 
अपने व पराये रसायनों व कीटाणुओं में भेद , जटिलता और शरीर-भर में उपस्थिति और अन्तःस्थ व अर्जित के रूप में दो प्रकार होना प्रतिरक्षा-तन्त्र की महत्त्वपूर्ण विशिष्टताएँ हैं। इस प्रतिरक्षा तन्त्र को न आसानी से समझा जा सकता है और न केवल प्रयासों से हमेशा स्वस्थ रखा जा सकता है। प्रतिरक्षा-तन्त्र काफ़ी हद तक हमारी आनुवंशिकी यानी जेनेटिक्स पर भी निर्भर रहता है। कोशिकाओं के भीतर स्वस्थ जीन ही आएँ , इसके लिए हम बहुत-कुछ कर नहीं सकते। नीचे बताये गये चार विषयों पर किन्तु हम ज़रूर ध्यान दे सकते हैं ; साथ ही आसपास के पर्यावरण से प्रदूषण को घटाकर प्रतिरक्षा-तन्त्र को स्वस्थ रखने का साझा प्रयास भी कर सकते हैं। 
 
1 ) सही और सन्तुलित भोजन का सेवन। 
 
2 ) निरन्तर शारीरिक व मानसिक व्यायाम। 
 
3 ) उचित निद्रा व तनाव से मुक्ति। 
 
4 ) नशे से दूरी। 
 
यह ध्यान रखना चाहिए कि जिस तरह से कोई केवल पढ़ने से पास नहीं हो सकता , उसी तरह केवल कोशिश करने से इम्यून सिस्टम को मज़बूत नहीं किया जा सकता है। क्योंकि पढ़ना पास होने की कोशिश है , पास होने की गारंटी नहीं। उसी तरह प्रतिरक्षा-तन्त्र को सही खा कर , ठीक से सो कर , नशा न करके, तनाव से दूर रहकर व व्यायाम द्वारा स्वस्थ रहने की केवल कोशिश की जा सकती है। 
 
व्यक्तिगत और सार्वजनिक पर्यावरण को यथासम्भव स्वस्थ रखना ही प्रतिरक्षा-तन्त्र के सुचारु कामकाज के लिए हमारा योगदान हो सकता है। आनुवंशिकी तो फिर जैसी है , वैसी है ही।

आखिर क्यों कुछ लोगों में वैक्सीन लगने के बाद भी एंटीबॉडी विकसित नहीं होती है, वैज्ञानिकों ने इस पहेली को सुलझा लिया है। उन्होंने इसके लिए मेटाबॉलिक हार्मोन 'लेप्टिन' की कमी को जिम्मेवार माना है। यह जानकारी यूनिवर्सिटी ऑफ क्वींसलैंड द्वारा किए एक शोध में सामने आई है जोकि जर्नल नेचर कम्युनिकेशन्स में प्रकाशित हुआ है।

शोधकर्ताओं ने यह खोज इन्फ्लूएंजा और हेपेटाइटिस बी वैक्सीन की प्रतिक्रियाओं की जांच करते हुए की है। यूनिवर्सिटी ऑफ क्वींसलैंड के प्रोफेसर डि यू ने मेटाबॉलिक और इम्यून सिस्टम के बीच की इस कड़ी का पता लगाया है। इसका उपयोग कमजोर प्रतिरक्षा वाली आबादी के लिए रणनीति विकसित करने के लिए किया जा सकता है, जिससे वैक्सीन उनपर बेहतर तरीके से काम करे।

प्रोफेसर यू ने जानकारी दी है कि इस अध्ययन में इम्म्यूनोलॉजी, जेनेटिक्स और बायोकेमिस्ट्री की उन्नत तकनीकों का उपयोग किया गया है। इस शोध से पता चला है कि लेप्टिन ने सीधे तौर पर कोशिकाओं के विकास और कार्य को बढ़ावा दिया है जो किसी एंटीबॉडी प्रतिक्रिया को शुरू करने में मददगार होता है।

उनके अनुसार अंतराष्ट्रीय शोधकर्ताओं के एक दल की मदद से हमने इस लेप्टिन नामक एक आवश्यक मेटाबॉलिक हार्मोन की पहचान की है जो वृद्ध और जवान दोनों में वैक्सीन की प्रतिक्रियाओं से जुड़ा था।

शरीर में फैट सेल्स से बनता है हार्मोन लेप्टिन

इसकी मदद से हम उन लोगों की पहचान कर सकते हैं जिनमें टीकाकरण के बाद भी एंटीबॉडी प्रतिक्रिया उत्पन्न नहीं होने का जोखिम सबसे ज्यादा होता है। यू ने बताया कि लेप्टिन नामक यह हार्मोन शरीर में मुख्य तौर पर वसा ऊतकों (फैट सेल्स) द्वारा बनता है। यह बात पहले से ही साबित है कि वैक्सीन अलग-अलग लोगों पर अलग मात्रा में असर करती है।

इसके साथ ही हमारी जेनेटिक्स भी इसपर आंशिक रूप से असर डालती है। इसके साथ ही अन्य कारक भी मायने रखते हैं। लेकिन इतना जरूर है जब हम फिट और स्वस्थ होते हैं, तो वैक्सीन कहीं बेहतर तरीके से असर करती है।

यू के अनुसार जब हम हम स्वस्थ होते हैं तो हमारा मेटाबॉलिस्म अच्छा रहता है और लेप्टिन का स्तर सामान्य रहता है। वहीं यदि कोई व्यक्ति कुपोषण या किसी अन्य बीमारी से ग्रस्त होता है तो उसमें लेप्टिन का स्तर घट सकता है, जो वैक्सीन की प्रतिक्रिया और इम्यून सिस्टम की सुरक्षा को सीमित कर सकता है।

उनके अनुसार कुछ लोग मोटापे से ग्रस्त होते हैं जिनमें लेप्टिन का स्तर सामान्य से ज्यादा होता है। जिस कारण लेप्टिन के कारण पैदा होने वाला प्रतिरोध, वैक्सीन के असर को प्रभावित कर सकता है। इस पर ध्यान देना जरुरी है। आने वाले समय में शोधकर्ता कोविड-19 वैक्सीन की प्रतिक्रियाओं का परीक्षण करने के इच्छुक हैं जिससे उन लोगों की पहचान की जा सके, जिनके शरीर में यह वैक्सीन  ठीक से असर नहीं कर रही है, इसकी मदद से बायोमार्कर निर्धारित किए जा सके।

उनके अनुसार कोरोना महामारी के इस दौर में सार्स-कोव-2 का सफल टीकाकरण ही समाज को सामान्य स्थिति में लाने की सबसे बड़ी उम्मीद है। ऐसे में बड़े पैमाने पर चलाए जा रहे टीकाकरण कार्यक्रमों में अलग-अलग लोगों में वैक्सीन की अलग-अलग प्रतिक्रियाएं अड़चन पैदा कर सकती हैं।

Is India's public health infrastructure ready to tackle the second COVID-19 wave? Here's what data says

Government hospitals in Maharashtra added just 672 ICU beds between April 2020 and January 2021

The novel coronavirus disease (COVID-19) has raged into a second wave which has become more testing in April: More than 273,000 news cases were recorded April 19, 2021 and 1,620 deaths.

This more deadly phase has stressed out India’s health infrastructure and the improvements made in the last year. Scarcity of testing kits, vaccines, hospital beds, medical oxygen, etc have been reported from several states. 

How well did we utilise the time and our learning since the outbreak began. Here are some statistics:

The Government of India approved a Rs 15,000-crore ‘COVID-19 Emergency Response and Health System Preparedness Package’ April 22, 2020 to boost India’s public health infrastructure sector.

The states have used this to increase the number of oxygen-supported beds, intensive care unit (ICU) beds and ventilators.

Oxygen beds grew 152%

Till January 2021, the country added 94,880 oxygen-supported beds. In April last year, the country had just 62,458 beds with oxygen, according to data presented by Ashwini Kumar Choubey, Union minister of state for health and family welfare in the Rajya Sabha Feb 2, 2021.

Maharashtra and Tamil Nadu accounted for 16,000 and 17,000 of these additional beds, respectively.

These figures seem substantial in absolute numbers but not so much when we compare them to the COVID-19 burden of these states at present.

Maharashtra was the worst-hit during both the major surges of COVID-19 cases in the country. On April 5, 2021, the state had 55 per cent of the country’s 103,558 active cases.

It also accounted for over 46 per cent of the total 478 deaths on April 5, 2021.

Tamil Nadu had 21,958 active cases on April 5, 2021.

In the same period, the number of oxygen beds in Delhi and Tripura increased 51 and 52 times, respectively — the biggest jump in the country.

The capital now has 5,977 such beds in comparison to just 115 in April, 2020. Tripura had just 10 oxygen-supported beds in April last year and now has 506.

ICU beds grew 32%

Intensive care unit (April 2020-January 2021)

India has 36,008 ICU beds at present compared to 27,360 in April 2020. Of these 8,648 additional beds, Maharashtra has been able to add merely 672 ICU beds — a growth of 14 per cent.

Delhi, on the other hand, added 1,861 ICU beds. The present total is six times that of last April levels.

A worrying signal

Even amid a second surge in cases, Haryana, Punjab, Puducherry, Goa, Himachal Pradesh, Rajasthan, Assam, Andhra Pradesh, Telangana and Jharkhand have decreased the number of ICU beds in their states.

Haryana and Punjab, where cases are rising steadily, have reduced ICU beds by 79 and 70 per cent, respectively.  These states have also reduced the number of ventilators by 73 and 78 per cent, respectively.

The second wave, attributed to the new coronavirus strains in the country, could be deadlier, said Dr Randeep Guleria, director of AIIMS, Delhi.

India added 10,461 ventilators since April 2020.

While 26 states, including Maharashtra, have strengthened their critical care infrastructure by adding ventilators, the number has reduced in nine states, according to health ministry data presented in Rajya Sabha Feb 2, 2021.

India’s healthcare spending is abysmally low. The country ranks second-last among countries in its region in terms of public health spending as a share of its gross domestic product (GDP), according to the World Health Organization. With less than one per cent public health spending as a share of GDP, India lags behind Bhutan (2.5 per cent), Sri Lanka (1.6 per cent) and Nepal (1.1 per cent), showed the National Health Profile 2019.

 ‘Manpower summary’

The state health department has prepared a ‘manpower summary of doctors in Haryana.’ It says as on May 1 this year, there are 2,748 doctors in place against the sanctioned strength of 3083. On the face of it, the situation does not appear grim. Scrape a little deeper and you’d find the mess:

  • Of the 2,748 doctors, nearly 300 have joined their duty, says the additional director. Sources said that over 500 doctors are either on maternity/childcare leave or are pursuing postgraduate courses and drawing salary from the government.
  • As many as 513 others are surplus, which means they are working at places where sanctioned posts don’t exist.
  • The doctors seem to have a particular penchant for working at Panchkula, as besides surplus doctors in the director office, 18 more doctors are working without a sanctioned post in this district. Sources say many of those working in Panchkula are related to politicians or bureaucrats or are well-connected.

In contrast, Bhiwani has 143 doctors against 222 posts, Fatehabad has 86 (129), Hisar 146 (214), Jind 92 (199), Kaithal 82 (132), Sonipat 128 (157) and Yamunanagar, 91 doctors against 112 posts.

Mandeep Kaur, additional director (administration), health department, says the number of doctors working in government hospitals and health centres is not static, but swings every month.

“We have recently recruited over 880 new doctors. Around 300 of them have already joined and several more are in the process of joining. However, this month, we will have to relieve some doctors who have got admission to PG courses. Similarly, some woman doctors proceed on childcare leave or maternity leave.” 

She says the situation is much better at the senior medical officers level. “The shortage is seen at the level of medical officers.”

New plan: More colleges

The government has taken up opening one medical college each in 22 districts. To encourage private efforts, it has been contacting business houses and, even religious leaders, as it may not be possible for the government to set up so many colleges on its own.

The official website of the Haryana Medical Education and Research Department shows the state has five government medical colleges, one government-affiliated college and five private medical colleges.

Health minister Anil Vij says the government has given an NoC to Meeri Peeri Medical College at Shahbad (Kurukshetra) to be run by the Shiromani Gurdwara Parbandhak Committee and the college is likely to come up very soon.

The government efforts to persuade Dera Sacha Sauda chief Gurmeet Ram Rahim Singh to set up a college in Sirsa came to naught after his conviction and sentencing in a rape case.

Sources say the health minister Anil Vij has also written to the Dera Beas, but the Dera has declined the offer. But Agarwal Samaj from Delhi has shown interest in opening medical colleges near Bahadurgarh in Jhajjar district. “We will soon achieve the target. It will go a long way in solving the shortage problem,” says Vij.

‘Won’t solve the problem’

Health department sources say opening new colleges will rather aggravate the problem. “Medical colleges do not run without doctors. Our existing medical colleges do not have enough doctors as faculty. Whenever there is an inspection, we receive a request from the Department of Medical Education and Research for deputing some more doctors there,” says a senior officer.

A private medical college representative, who does not want to be identified, says the government “whimsically” changes policies. “Private medical colleges have to charge a fee decided by the government which is often inadequate to meet the expenditure on salary of doctors and infrastructure”, he said.

Sources say a Faridabad-based medical college was shuttered by its owners in 2016 because it was unable to run it with the funds the college generated.
